Cancellation Policy

NO SHOWS/ NO CALLS/ LATE FEES: Pursuant to Body Werk Training Services Only. Body Werk Training client appointment sessions without clients attendance due to a no call/no show/ non-mindbody unbooking or contacting of the studio to perform cancellation result is the following fees to account to be paid prior to next SERVICE rendered. a) NO CALL/ NO SHOW [$9.99] b) NON-UNBOOKING OF SESSION OR NOTIFICATION [$9.99] c) LATE ARRIVAL BEYOND 15 MINUTES [$9.99] SESSION EXPIRATIONS: Purchases of packages include sessions, in which expirations apply. Expirations are outlined below:  SOLO EXPERIENCE [7 DAY EXP]  SOLO SPECIAL EXPERIENCE [7 DAY EXP]  (3) GROUP SESSIONS [10 DAY EXP]  (5) GROUP SESSIONS [15 DAY EXP]  (10) GROUP SESSIONS [30 DAY EXP]  (10) GROUP SESSIONS [20 DAY EXP]  (15) GROUP SESSIONS [30 DAY EXP]  MONTHLY MEMBERSHIP [30 DAY EXP]  (1) BWT TRAINING SESSIONS [7 DAY EXP]  (3) BWT TRAINING SESSIONS [14 DAY EXP]  (4) BWT TRAINING SESSIONS [14 DAY EXP]  (6) BWT TRAINING SESSIONS [30 DAY EXP]  (8) BWT TRAINING SESSIONS [30 DAY EXP]  (10) BWT TRAINING SESSIONS [30 DAY EXP]  UNLIMITED BWT TRAINING SESSIONS [30 DAY EXP] CANCELLATIONS | CANCELLATION FEE: I, agree and understand that this gym membership contract requires a (30) THIRTY DAY cancellation notification for any and all SERVICES, in which services will be charged for the next month without cancellation. If the gym membership contract is cancelled prior to initiation of SERVICES, up to including (2) days of said purchase, member agrees to pay a cancellation fee of ($49.99) forty dollars and ninety nine cents for service processing. If cancellation is as a result to military services or relocation to an area more than 25 miles from the studio, cancellation without a cancellation fee will be permitted. Cancellation notices are to be transmitted in writing to, Queen’s Studio 5 Project at Queens@queensstudio5.com or 1621 Fulton Avenue, Suite 23, Sacramento, California 95825. All Classes/Session(s) are to be cancelled (2) two hours prior to session or class. Should session not be cancelled within the said time constraints, said session is lost and to be debited from pack count.
